1. Refund Policy

Refunds will only be issued under the following conditions:

• The appointment is canceled at      least 24 hours prior to the scheduled appointment time; or
• A verified error or fault by our facility has occurred.

Failure to provide at least 24 hours’ notice will result in the forfeiture of any fees paid. Refund eligibility is determined at the sole discretion of our facility based on the circumstances provided.

2. Confidentiality & Privacy Policy

We take confidentiality and data protection very seriously.

• All testing, services, and personal information are kept strictly confidential.
• We comply with all applicable privacy laws to safeguard your personal and medical information.
• Test results and related information will only be released to:
  • The donor
  • The Medical Review Officer (MRO)
  • The Designated Employer Representative (DER)
• Results will be sent only to the email address or physical address provided by the donor.
• Our facility will not copy (CC), forward, or release any results or personal information to individuals or entities not explicitly listed in the contact information on the Chain of Custody Form.
• Unauthorized requests for information will be denied to protect your privacy.

3. Information Accuracy

It is the responsibility of the donor or client to ensure that all personal and contact information provided is accurate and up to date. We are not responsible for misdirected communications due to incorrect or incomplete information.

4. Appointment Policy

• Clients are expected to arrive on time for scheduled appointments.
• Late arrivals may result in rescheduling at our discretion.
• No-shows may be subject to fees or denial of future service.

5. Service Limitations

Our facility provides testing and related services in accordance with industry standards and regulatory guidelines. We do not guarantee specific outcomes or results beyond accurate processing and reporting.

6. Liability Disclaimer

While we strive for accuracy and professionalism, our facility shall not be held liable for:

• Delays caused by third-party laboratories or external factors
• Consequences resulting from the use or interpretation of test results
• Issues arising from incorrect information provided by the client

7. Policy Updates

We reserve the right to update or modify these policies at any time without prior notice. Changes will be effective immediately upon posting on our website.
